I've only entered Ukraine once, by plane from Moscow, in 2008.

At Kiev airport Entry Controls I just showed a UK Passport, was asked how long I was there for and where I was staying. I cannot recall now but there may have been a simple Landing Card like nonEU citizens have to complete when entering UK (shorter than me entering USA :p )
The passport was stamped with the place and date of entry. Nothing else. No forms. No visa. No fees!
