Regarding the 1-month 'visa-on-arrival extension', yeah this can be done on bali through a 'travel agent', its openly advertised and will cost you between 1200000 - 1500000 Rupiah...

I'm in Bali and just got 30-day extension thru an immigration company for US$200. Its private free enterprise, I do not know how they do it, do not care, but its totally legit and came recommended by others who have used them. $200 is a lot for 30 days but cheaper than flying somewhere to get new visa. There are services that do it for $150 evidently.
