On the fiche there is a line for your home address.

We are working in Sierra Leone and have Sierra Leone drivers licenses. To avoid confusion is the best address to use the one on the drivers licenses or should we use one from Canada where we are citizens? Does it really matter?

We'll be driving a Sierra Leone plated vehicle registered in our name.

I also have the Canadian registration document and licenses if that is better but I can only imagine that would complicate things.

Also, how confusing will it be for these guys when ones place of birth does not match ones country of citizenship? In Guatemala when we applied for residency they had a tough time with one holding citizenship of a country other than the birth country.

I would use the Sierra Leone address as that's the one on the documents, just avoid questions. One thing we learned in crossing over 30 African borders to date is that no one really checks up on details on what you fill in on forms, so as long as it looks plausible.
